Please completely fill out the Final Service Request, sign and submit to our office.

You cannot select a move out date prior to today’s date, weekend date or a holiday.

For your convenience, the application and copies can be submitted via the following:

  1. Faxed to (949) 858-1617.
  2. Scanned and e-mailed to customerservice@tcwd.ca.gov.
  3. In person to our office between 7:00 a.m. and 3:30 p.m. normal business days (Monday - Friday).
  4. Dropped off in one of the two drop boxes located in our parking lot.
After the completed application is received, we will generate a Final Service Confirmation form, which requires an additional signature before service will be changed.

The confirmation form will include your final read date, final bill date, and final bill due date.
